About OASAS Addiction Treatment Centers Creedmoor Addiction Treatment Center

Creedmoor Addiction Treatment Center (ATC) is a facility operated and certified by New York State Office of Alcoholism and Substance Abuse Services and is accredited by the Commission on Accreditation of Rehabilitation Facilities.
The ATC opened in 1974 and maintains 26 beds for adult men and women.
It is an integral part of the alcohol and substance abuse service delivery system in Queens, and plans its services to be responsive to the community need.
Services are provided which address the basic health care needs of each individual while addressing specific physical, psychological, and social symptoms associated with addictive disease.



Criteria for AdmissionCreedmoor ATC Screening Form Fillable PDF Form   
The individual appears to be in need of inpatient chemical dependency services.     The individual appears not to be in need of a higher level of care.     The individual is unable to participate in or comply with treatment outside of a 24 hour structured treatment setting based on one or more of the following:        a living environment not conducive to recovery,        physical or mental complications and co-morbidities requiring medical management,        a lack of judgment, insights, and motivation such as to require 24 hour supervision.    

Final admission decisions are always made by the ATCs medical director in compliance with utilization review standards.   
Individual must reside in catchment area (county covered by ATC) listed in directory.    
The individual must be at least 18.
Under special circumstances, a person under 18 may be admitted.

Fees based on Individual's ability to pay (No individuals denied access based on inability to pay)ServicesCreedmoor Addiction Treatment Center is a 26 bed, short term inpatient addiction rehabilitation program which is medically supervised and staffed by a multi-disciplinary treatment team of professionals.
Treatment is provided in settings which are comfortable, home-like, and which encourage discussion among and between staff and clients. 
Individualized treatment planning addresses all aspects of personal recovery.
Services include group, individual and family counseling, specialty groups, comprehensive client education series, medical, psychiatric and psycho-social intervention and an extensive activities therapy program.
A variety of Self Help Groups are offered as an adjunct to treatment.
The importance of continuing involvement in support groups is encouraged, with meetings offered both on the unit and in the community.
Participation in all aspects of the program is required.
The treatment which is planned involves a broad array of clinical and educational experiences designed to assist each individual achieve their potential.
Visitors and phone calls are encouraged during designated hours, with the permission of the client.
Visitors must observe their policy of providing an alcohol and drug free setting.


Visiting Hours:
Once monthly, the last Saturday of each month -- 1:30 pm - 3:30 pm

ELIGIBILITY:
Services are available to individuals who are in need of inpatient care for addiction and reside in the county of Queens. Individuals served must be free of all alcohol and drugs for a minimum of 48 hours prior to screening . They must not be in need of detoxification, other acute medical services, or primary psychiatric service. Clients are admitted on a voluntary basis only. Individuals must be able to climb stairs without assistance and be willing to cooperate with all program rules and regulations, which are explained in the Client Handbook. Medicaid and other third party insurance accepted. Sliding scale fees are based upon eligibility to pay. No individual is denied service because of financial situation. Creedmoor ATC maintains a Tobacco free environment.

SPECIAL POPULATIONS:
Creedmoor Addiction Treatment Center offers specialized services to addicted women through a women's program which builds on the identification of the strengths and needs of the women. The program fosters a safe, nurturing and supportive environment designed to address the barriers women encounter throughout their treatment experience. Another unique focus of treatment at the Creedmoor ATC includes services for homeless men. The program offers a full array of medical services, linkages and referrals to continuing care and housing assistance in addition to addiction treatment. They recognize the stigma which is often attached to special populations, and assist individuals in developing ongoing care plans which address their special needs. All persons admitted receive a discharge plan which includes recommendations for continuing care.

PROBLEM GAMBLING:
Individual and group counseling for those affected by problem gambling. Clinical staff on site have specialized problem gambling training and/or credentials and are able to provide direct clinical services as both an individualized issue and as a co-occurring disorder. Patients do not have to have a primary substance abuse diagnosis to be admitted for problem gambling treatment services. To make a referral or for more information about problem gambling inpatient treatment, please call the Admissions Office at 718-264-3742.

REFERRAL PROCESS:
Individuals should call during business hours Monday through Friday, 718-264-3740, and ask for a screening. General information will be taken to determine eligibility and appropriateness for service. Referring agencies should prepare a complete assessment of the client, including a detailed history of substance abuse. Referral agencies who wish to be notified about the client's admission or general status should obtain a consent to release information from the client as without proper written consents, no verbal or written information will be released. Client records are confidential and any disclosure of client identity or other information is made in accordance with applicable Federal and State law.

I'm a pretty complicated case, but i have been looking for treatment i could afford with $0 in my bank account, and there are not a lot of options for that, or they're just hard to find. But i just need help to get out of where i've been. i just need help battling my addictions, but the one thing im afraid of is going to rehab and taking drugs, because ive never taken any drugs except an Advil maybe once. But i just need help
